Print out and complete this form. You may return this form through the Sunday Collection, drop off in the parish office, mail or fax to us.
Payment Authorization Form

Space Use Request Form
This form needs to be completed to request the use of space at St. Mary's. Be sure to check the calendar to make sure the space you are requesting is not already in use.

Request for Baptismal Certificate
A copy of the requested baptismal certificate will be sent to you (unless you note otherwise) upon the completed form being returned to the Parish Office.
